PSPI v3 Voltage Issue

I'm only getting a voltage of 4.06v for both the pi and the screen.  The screen has no issue running off this but it's an issue for the pi as it's not booting and setting pin 14 properly.  Is there something I've missed that is causing the low voltage?  The wall adapter is pushing 5.36v and the batteries are fully charged at 4.13v.  If it's not plugged in nothing happens when I switch it on.

No, it should just work. 4.06v to the LCD is very strange. Also, the LCD shouldn't function at all with less than 4.5v, so that's confusing me further.

How do you feel about sending the whole thing to me (case and all) so I can troubleshoot it? 

Edit:

Double check your battery connections. Also what voltage are you getting from the batteries when the wall adapter isn't plugged in. This will happen if the batteries aren't fully connected.

I Re-re-re soldered the batteries until I had a more solid connection, that fixed the voltage to the pi and the lcd.  

However it seems my pi0w is bricked, I've tested the sdcard on a standard pi0 and it works fine, but I'm not even getting the LED to light up.  

Edit: I've ordered some soldering wick to attempt to remove the "broken" pi and test it / put in the working pi0

Use lots of flux too. I had to do the same thing on a test build, and it's not easy. Hopefully you dont destroy the board in the process, but let me know if you do and I'll see what I can do to help.
